Qualidigm’s long term care consultants help nursing homes identify areas of focus and bring their knowledge and expertise in care planning, understanding QIs/QMs, dementia care, falls, culture change, and other areas to improve patient safety and care for all residents.

Our consultants have many years of experience working collaboratively with nursing home staff at all levels, and understand the unique goals and challenges facing long term and post-acute care providers today.

By a process of data analysis, evaluation and assessment, Qualidigm’s long term care team assists organizations to envision and implement changes and improvements that are based on real data and evidence-based practices. We also offer specialized training and education with the nationally-approved TeamSTEPPS® for Long Term Care and INTERACTTM tools.
